Permission for a minor to leave Chile

(valid for 180 days only)

If you live far away from Vancouver and can't come to our office, you can sign a written travel consent in the presence of a Notary Public and have it legalized according to the procedure outlined on LEGALIZATIONS.

A sample letter that you can modify to reflect  your particular case can be downloaded here.

To obtain this document the parent(s) that is (are) not traveling must appear in person and bring the following documentation:

Passport or drivers licence (MUST BE CURRENT)

Proof of paternity: Original birth certificate of the minor showing the names of the parents

Passport number of the minor

Full name and passport number of the person(s) the minor is traveling with (if applicable)

Fee: US$ 7.-

Please have this document legalized according to the procedure outlined on LEGALIZATIONS. Without proper legalization it will not be valid in Chile!
