How to officially register the death of a Chilean citizen

1) Obtain a Certificate of Death from the  Department of Vital Statistics of the province where the death ocurred. 

2) Consular registration of death. The following documents are required:

a) Certificate of Death

b) Pasaport or ID Card of Chilean citizen who passed away (photocopy)

c) Copy of the Chilean birth certificate

d) Cause of death (medical certificate of death, original

e) Fee of US$ 17. 

Por persons that live at a great distance from the Consulate General:

You must send the above mentioned documents and an XPRESSPOST envelope or large envelope with enough stamps to send you the forms that must be signed and returned to the Consulate Genera.

If you want to sent human remains to Chile you must (MANDATORY):

a) Register the death with the Consulate General  

b) Obtain from the funeral home: 

1. A "certificate of cremation", if applicable,

2. A health certificate stating that the urn or casket is hermetically sealed and that the transportation of it is safe and there is no risk for anyone. In Vancover, B.C., this certiificate must be requested from the Coastal Health Department. In other cities please contact your local Health Department. Make sure that a sample signature of the signing official has been provided to the Consulate General.

3. Invoices for services provided by the funeral home to obtain a reimbursement either by the IPS (ex-INP) or the AFP (private pension plan administration), if you are applying for reimbursement of costs only.

4. Obtain from the Consulate General of Chile a legalized death certificate, see LEGALIZATIONS section. The cost of a legalization (authentication) is US$ 12, payable per money order.

For the requirements 1 and 3, the certificates or documents obtained MUST be signed in the presence of a Notary Public and afterwards authenticated by the Department of Justice of the province.  The documents 1, 2 and 3 must be legalized (authenticated) by the Consulate General of Chile (US$ 12 each).
